For this coverage you should have a membership card for yourself and each of your covered dependents.

Vision Benefits

Vision coverage is available through Davis Vision, you can find information by visiting www.davisvision.com
For this coverage you would not have an insurance card - you would tell the provider Davis Vision and use your social.

Dental Benefits

Effective 01-01-2014 South Eastern School District will offer dental coverage through Delta Dental. Please see the attached highlight sheet.
For this coverage you would have a card for each member but it is not required- you can tell the provider Delta Dental.
Life Insurance Benefits

All full time employees of South Eastern School District receive a $40,000 life insurance policy, at no cost to the employee, through Fortis.
If you need to update your beneficiaries - you can do so by forwarding the following form to Human Resources - your prior form on file will be replaced.
Life Insurance Form - you can use this form to submit to HR if you would like to update your beneficiaries that are currently on file.
